Management of Infants with Intestinal Failure: State of the Art on Nutrition Support and Intestinal Rehabilitation

Target Audience: Neonatologists, gastroenterologists, dietitians, pediatric surgeons, and basic scientists involved in the clinical management or research related to the management of infants with intestinal failure.
 
Objectives: - Identify the neonates and infants at high risk for intestinal failure[br]- Describe the pathophysiologic consequences of intestinal failure[br]- Discuss medical treatment modalities and a management approach for intestinal failure[br]- Discuss the indications for surgical interventions which may be beneficial to individual neonates and infants with intestinal failure

The incidence of neonates and infants with complex nutritional needs is increasing in westernized countries due to an increase in gastroschisis and the improved acute management of infants with severe Necrotizing Enterocolitis. This increasing population of babies with anatomical or physiological short bowel poses a significant challenge to neonatologists, gastroenterologists and general pediatric surgeons. The prolonged hospitalization includes intensive care and complex subspecialty care, in the context of a relatively frequent staff turnover and a paucity of evidence-based guidelines. In the past it has been documented that the use of a consistent multidisciplinary team improves outcomes in these complex infants. In this symposium we will review the evidence for an intestinal failure team, considering both local outcomes and experience across Canada. We will also focus on treatment modalities, including novel therapies such as alternate lipid emulsions and GLP2, and provide recommendations based on pathophysiology and mechanisms of action.
