| The primary care medical home has evolved substantially since its description over four decades ago. Recent health reform discussions have broadened the discussion to include adult medicine. Pediatric discussions have centered around investment in prevention and care of growing numbers of children/youth with chronic illness, as well as the application of the medical home model to all children. Changing child and family characteristics influence the design of the pediatric medical home.[br]Common pediatric and adult approaches to the patient/family-centered medical home include care coordination, community-oriented, communication, continuity of transitions, family-centered, comprehensive, cultural competence, compassionate, and communication. The pediatric family centered medical home nonetheless has some unique aspects, reflecting differential epidemiology and dependency. Active health care reform discussions address financial viability and feasibility. This symposium will highlight the evolution of the patient/family-centered medical home in pediatrics with an emphasis on history, the evidence base for medical home, and recent policy debates. |
